The Home Health Physical Therapist (PT) is responsible for providing therapeutic care to patients in their homes to improve mobility, reduce pain, and promote overall physical function. The PT assesses patient needs, develops individualized care plans, and delivers evidence-based treatment to support recovery and independence.
Conduct in-home physical therapy assessments and evaluations.
Develop and implement personalized treatment plans based on physician orders and patient goals.
Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
Educate patients and caregivers on safety, use of assistive devices, and home exercise programs.
Monitor and document patient progress, outcomes, and care modifications in the EHR.
Collaborate with nurses, occupational therapists, speech therapists, and case managers.
Ensure timely communication with the interdisciplinary team and referring physician.
Comply with all regulatory standards, safety protocols, and documentation requirements.
Education: Doctorate or Master’s degree in Physical Therapy from an accredited program
Licensure: Current PT license in [State]
Certifications: BLS required
Experience: 1–2 years of PT experience preferred; home health experience is a plus
Strong patient communication, clinical reasoning, and organizational skills
Reliable transportation and valid driver's license required for home visits
